Every student-athlete must complete these steps before their first practice. Registration is fully digital through FinalForms.

  Enroll in Skyward — must be an active MHS student or incoming 9th grader enrolled in Qmlativ/Skyward.

  Complete FinalForms — all parent and student signatures on waivers and participation documents required.
• 3
  Upload a valid physical — sports physical exam must be current and uploaded to FinalForms.
• 4
  Look for green — your name turns green in FinalForms when you're cleared to participate.
